When To Keep Your Child At Home:

* Vomiting within the last 24 hours
* Diarrhea within the last 24 hours
* Temperature of 100 or higher within the last 24 hours without anti-fever medication
* Any symptoms of acute illness such as persistent cough, runny nose with body aches, draining eye, or rash

Your child should look and behave like him/herself for 24-48 hours before returning to school. Why??

* To protect your child from exposure to more infection before s/he is able to build resistance.
* To protect the other children and school staff from communicable disease that your child may transmit.

If your child sees his/her Primary Care Provider or other Health Care Practitioner and misses school, please obtain a doctors note stating the reason for missing school, and that they are not contagious.

IMMUNIZATION REQUIREMENTS FOR PRE-K STUDENTS...

A signed certificate of immunization by a physician on physician's letterhead, indicating specific dates of quantities as stated below:

* 4 doses of DTaP

* 3 doses of Poliovirus (IPV/OPV) or four doses of enhanced inactivated poliovirus vaccine (EIPV)
* 1 dose of Measles, Mumps, and Rubella vaccine administered on or after 12 months of age

* 1 dose of Varicella (chicken pox) administered on or after 12 months of age
* 3 doses of Hepatitis B vaccine
* Haemophilus influenza type b (Hib) - 3 doses (or 1 dose if administered on or after 15 months of age)
*Pneumococcal PCV- 3 doses (or 1 dose if administered on or after 24 months of age)
* Blood tests which show immunity against measles, mumps or rubella are also accepted as proof of immunity

NOTE: If you are completing a kindergarten registration at this time and the second measles or varicella immunization has not yet been given, we can accept an appointment slip indicating the date the shot will be given. However, the appointment date must be in writing and signed by the physician. The date specified must be prior to the start of kindergarten and once administered, the proof of these immunization must be brought to the designated school or kindergarten entry will be denied.

* 5 doses of diphtheria -containing toxoid (usually administered DTP, DT or Td) unless 4^th dose was given after 4 years of age

* 4 doses of Polio (IPV/OPV)- unless 3^rd dose was given after 4 years of age

* 3 doses of Hepatitis B vaccine

* 2 doses of Varicella vaccine

* 2 doses of Measles, Mumps, and Rubella

* Haemophilus influenza type b (Hib) - 3 doses (or 1 dose if administered on or after 15 months of age)

*Pneumococcal PCV- 3 doses (or 1 dose if administered on or after 24 months of age)

Cuando debieras mantener su hijo/a en la casa:

* Vómitos en las últimas 24 horas

* Diarrea en las últimas 24 horas

* Temperatura/calentura de 100 o más alto en las últimas 24 horas sin medicación contra la fiebre

* Cualquiera de los síntomas de la enfermedad aguda como tos persistente, secreción nasal con dolores en el cuerpo, ojos llorosos, o una erupcion cutanea

Su hijo debediera estar actuando como si mismo por 24 a 48 horas antes de regresar a la escuela.

Por que??

* Para proteger a su hijo de la exposición a más infección antes de que sea capaz de establecer su Resistencia

* Para proteger a sus companeros y el personal de la escuela de enfermedades transmisibles que puede transmitir su hijo

** Si su hijo tiene un visita con su medico/ pediatra y no viene a la escuela por favor obtenga una nota del doctor indicando el motivo por el que falta la escuela y que no son contagiosos.

POR FAVOR TENGA EN CUENTA: Si estás completando la registración del Jardín de Kinder ahora y la segunda vacuna del Sarampión o Varicela no ha sido administrado, aceptamos una nota indicando la fecha que su hijo la va a recibir. LA NOTA TIENE QUE SER ESCRITA A MANO y FIRMADA POR EL MEDICO.

La fecha especificada debe ser antes del comienzo del Jardín de Kinder. Una vez administrado, la prueba de esta vacunación debe ser traída a la escuela designada o negarán la entrada al Jardín de Kinder.

REQUISITOS DE VACUNACION PARA JARDIN DE KINDER

* 5 dosis de la DTaP- menos que la 4^th dosis se administer a los 4 anos de edad

* 4 dosis de la Polio- menos que la 3^rd dosis se administer a los 4 anos de edad

* 3 dosis de la vacuna Hepatitis B

* 2 dosis de la vacuna de la Varicela

* 2 dosis de la vacuna Sarampion

* Análisis de sangre el cual demuestra inmunidad contra el sarampión, las paperas y la rubéola también son aceptados como prueba de inmunidad.
